Occasionally we play a room where a sign says "Occupancy 100 persons" and there might be 150 or even 200 people in there. What to do?

Seriously consider that if they don't pay attention to the fire code limit on occupancy, they aren't paying attention to other things. It might be a great room and it might pay well, but in an emergency.. I bet you are on your own.

Kinda hard to blame the venue for the band crew's irresponsible use of pyrotechnic devices. Of course they (club owners) may be legally responsible to compensate victims, but it certainly isn't their fault that it happened.

This was exactly my thought. Room capacities of public places are set by the fire department, for the purpose of safety during emergencies.
If a venue owner is stuffing the room, it means that money is more important to them then the safety of anyone in the room, including YOU.
